Surgery is considered for significant persistent symptoms, prolapse or external disease unsuitable for office treatment, or inadequate benefit from appropriate earlier care. Incidental hemorrhoids do not mandate surgery.

What should happen first?

Confirm the symptom source and assess evacuation, previous treatment, blood thinners and continence. Difficulty emptying despite soft stool may need separate assessment rather than expecting surgery to resolve it.
