What does grade IV mean?
Internal hemorrhoids remain prolapsed and cannot be reduced. An external lump alone does not establish this grade; it may be a skin tag, external hemorrhoid or rectal prolapse. Examination identifies the components.
Why is surgery often discussed?
Improving constipation and irritation may ease symptoms without correcting fixed prolapse. Surgery depends on symptom burden, external disease, overall health and preferences rather than the grade label alone.
What should not wait?
Severe pain, darkening prolapsed tissue or heavy bleeding needs urgent assessment. Do not force painful tissue back. Discuss internal and external treatment, recovery, narrowing and continence risks.
