When is surgery considered?
Poor response to appropriate therapy, precancerous changes, cancer or acute complications may lead to surgery. Gastroenterology and surgery coordinate care; elective and emergency pathways differ.
Permanent stoma or an internal pouch?
Options may include removal of colon and rectum with a stoma or an ileal pouch in suitable patients. A pouch is not a new colon with identical function; frequency, inflammation, continence and staged operations need discussion.
What deserves early planning?
Discuss nutrition, medicines, stages, stoma support, work, fertility and sexual function as relevant. Do not stop steroids or immune treatment yourself. Severe pain and distension with fever or heavy bleeding needs urgent assessment.
