A clot in an external hemorrhoid can cause a very tender, sometimes bluish lump. This differs from prolapsed internal hemorrhoids and an abscess. Examination matters, especially with fever or systemic illness.
Does it need removal?
Pain severity, onset, improvement and examination guide conservative care versus a procedure in selected cases. A time cutoff is not a universal rule. Never cut the lump yourself.
How should I monitor recovery?
Record onset and whether pain improves. A residual skin tag does not necessarily mean a persistent clot. Increasing pain, fever or spreading swelling requires prompt reassessment.
